Passing an explicit filename to this command is only necessary if the
documentation in the @file block refers to a file different from the
one the block resides in.

replace ugly macros by always_inline functions, that way its much more readable and flexible as always_inline can simply be removed while the macros couldnt be
about 0.5 % speedup with default parameters
