The majority of frame-threaded decoders (mainly the intra-only)
need exactly one part of ThreadFrame: The AVFrame. They don't
need the owners nor the progress, yet they had to use it because
ff_thread_(get|release)_buffer() requires it.
This commit changes this and makes these functions work with ordinary
AVFrames; the decoders that need the extra fields for progress
use ff_thread_(get|release)_ext_buffer() which work exactly
as ff_thread_(get|release)_buffer() used to do.
This also avoids some unnecessary allocations of progress AVBuffers,
namely for H.264 and HEVC film grain frames: These frames are not
used for synchronization and therefore don't need a ThreadFrame.
Also move the ThreadFrame structure as well as ff_thread_ref_frame()
to threadframe.h, the header for frame-threaded decoders with
inter-frame dependencies.

Given that the AVCodec.next pointer has now been removed, most of the
AVCodecs are not modified at all any more and can therefore be made
const (as this patch does); the only exceptions are the very few codecs
for external libraries that have a init_static_data callback.

The current design, where
- proper init is called for the first per-thread context
- first thread's private data is copied into private data for all the
other threads
- a "fixup" function is called for all the other threads to e.g.
allocate dynamically allocated data
is very fragile and hard to follow, so it is abandoned. Instead, the
same init function is used to init each per-thread context. Where
necessary, AVCodecInternal.is_copy can be used to differentiate between
the first thread and the other ones (e.g. for decoding the extradata
just once).
Clipping was added in 2009 to avoid crashes.
The clipped case would produce a 2nd slice with 0 input
thus also producing 0 output.
Subsequent checks will cause decoder failure unless both
slices have the same output length. thus the only way this
would not already fail is if the output from both slices
was 0 bytes.

Other dimensions would not work correctly currently,
also ask for a sample for files that fail this check.
This fixes an integer overflow leading to out of array
accesses.

Sample from ticket #1216 does not use any compression but bitstream
claims it is. Reference decoder decodes sample just fine.

This prevents crashes when trying to read beyond the end of the buffer
while decoding frame data.

If the PNG filter is enabled, a PNG-style filter will run over the
input buffer, writing into the buffer. Therefore, if no zlib compression
was used, ensure that we copy into a temporary buffer, otherwise we
overwrite user-provided input data.
This is required specifically for setting frame->format to -1,
otherwise it will be set to 0 = PIX_FMT_YUV420P and code reading
the format from the output decoded frame will get misled.
In particular fix regressions occurring with the pending vsrc_buffer
patch.
None of these symbols should be accessed directly, so declare them as
hidden.

Passing an explicit filename to this command is only necessary if the
documentation in the @file block refers to a file different from the
one the block resides in.
