Removes the special -I flag specified in the avcodec/bsf/ subdirectory.
This makes code copy-pastable to other parts of the ffmpeg codebase, as
well as simplifying the build script.
It also reduces ambiguity, since there are many instances of same-named
header files existing in both libavformat/ and libavcodec/
subdirectories.

There are lots of files that don't need it: The number of object
files that actually need it went down from 2011 to 884 here.
Keep it for external users in order to not cause breakages.
Also improve the other headers a bit while just at it.

It hides the internal part of AVBitStreamFilter by adding a new
internal structure FFBitStreamFilter (declared in bsf_internal.h)
that has an AVBitStreamFilter as its first member; the internal
part of AVBitStreamFilter is moved to this new structure.

If a sequence header has already been found, it is certain that next
startcode (being disjoint from the sequence header startcode) can begin
at index four at the earliest.
