Given that the AVCodec.next pointer has now been removed, most of the
AVCodecs are not modified at all any more and can therefore be made
const (as this patch does); the only exceptions are the very few codecs
for external libraries that have a init_static_data callback.

The decoders in this set either have a fixed channel count, or read it
from the bitstream, and thus do not require the channel count as
external information.
Fixes various regressions since
81503ac58a, which requires a valid channel
count for decoders which do not set this capability.

As is the decoder will never stop, it will cause an infinite loop. The RFC seems only
to speak of non empty packets so endlessly generating noise from the last empty flush
packets seems wrong.

Without this its possible to make the new decode API decode billions of samples
out of a empty input and never return to the caller before all samples have been
created and discarded.

The idea is to use ffmath.h for internal implementations of math functions.
Currently, it is used for variants of libm functions, but is by no means
limited to such things.
Note that this is not exported; use lavu/mathematics for such purposes.

After a flush, don't average the output envelope and energy with
previous iterations.
Also start directly from the target values for the first iteration
at startup.

This isn't too useful as a normal codec, but can be used in
voip style applications. The decoder updates the noise
generator parameters when a packet is given to it for decoding,
but if called with an empty packet, it generates more noise
according to the last parameters.
