Removes the special -I flag specified in the avcodec/bsf/ subdirectory.
This makes code copy-pastable to other parts of the ffmpeg codebase, as
well as simplifying the build script.
It also reduces ambiguity, since there are many instances of same-named
header files existing in both libavformat/ and libavcodec/
subdirectories.

In case the BSF has not been drained before flushing/closing,
the context's next_frame might be set; yet it is not freed
in flush or close. The former only zeroes it (which automatically
causes a leak in case it was set). So do this when closing
and flushing.

It hides the internal part of AVBitStreamFilter by adding a new
internal structure FFBitStreamFilter (declared in bsf_internal.h)
that has an AVBitStreamFilter as its first member; the internal
part of AVBitStreamFilter is moved to this new structure.

This bsf doesn't have any options, so including libavutil/opt.h is
unnecessary.

Takes a raw input stream containing frames with correct timestamps but
possibly out of order and inserts additional show-existing-frame
packets to correct the ordering.

Also converted from bitstream to get_bits.
This was left over from an earlier version which created the new
packet inside the current frame structure. Now it just leaks an
unused packet, so remove the allocation entirely.
