It is only used by encoders; in fact, AVCodecContext.time_base
is only used by encoders, so it is only useful for encoders.
Also constify the AVCodecContext parameter in it.
Also fixup the other headers a bit while removing now unnecessary
internal.h inclusions.

Note that convergence_duration had another meaning, one which was in
practice never used. The only real use for it was a 64 bit replacement
for the duration field. It's better just to make duration 64 bits, and
to get rid of it.

Currently, the amount of padding inserted at the beginning by some audio
encoders, is exported through AVCodecContext.delay. However
- the term 'delay' is heavily overloaded and can have multiple different
meanings even in the case of audio encoding.
- this field has entirely different meanings, depending on whether the
codec context is used for encoding or decoding (and has yet another
different meaning for video), preventing generic handling of the codec
context.
Therefore, add a new field -- AVCodecContext.initial_padding. It could
conceivably be used for decoding as well at a later point.
"que" sounds like a slang word to me. This commit renames a few
variables, fix the comments and the logging messages (sometimes along
with small other typo fixes).
This fixes building with DEBUG defined after the function was made
static and the prototype removed in d7f9786cbc.

Also update libav->ffmpeg as theres pretty much no code left from libav.
The new code is faster, requires fewer mallocs and less memory. Its
also half the number of lines of code.
This code is not 100% identical in behavior to the previous, but the
differences appear to be rather limitations of the previous design
than intended though i could be wrong of course.
