The internal line accumulator for 16bit can overflow, so I changed that
from int to uint64_t in the C code. The matching assembly looks a little
weird but output looks correct.
(avx2 should be trivial to add later.)

This way, the special IDCT permutations are no longer needed. Bfin code
is disabled until someone updates it. This is similar to how H264 does
it, and removes the dsputil dependency imposed by the scantable code.

Passing an explicit filename to this command is only necessary if the
documentation in the @file block refers to a file different from the
one the block resides in.

Consistently apply this rule: the guard name is obtained from the
filename by stripping the leading "lib", converting '/' and '.' to
'_' and uppercasing the resulting name. Guard names in the root
directory have to be prefixed by "FFMPEG_".
