Improved DVB subtitles encoder to generate AVPacket.data in the same
format as generates MPEGTS demuxer + DVB subtitles parser. So now single
format of DVB subtitles data is used across all the components of FFmpeg:
only subtitles payload WITHOUT 0x20 0x00 bytes at the beginning and 0xFF
trailing byte.
Improved MPEGTS muxer to support format of DVB subtitles in
AVPacket.data described above: while muxing we add two bytes 0x20 0x00 to
the beginning of and 0xFF to the end of DVB subtitles payload.
The patch fixes DVB subtitle copy problems: tickets #2989 fully and #2024
partly.

Many video players accept broken frames in a transport stream, so there
is no reason to abort remuxing when encountering one, just print a
warning instead.
Fixes ticket #1758.
Also move the declaration to internal.h, and add restrict qualifiers
to the declaration (as in the implementation).

Make the muxers/demuxers that use the field handle the default
-1 in the same way as 0.
This allows distinguishing an intentionally set 0 from the default
value where the user hasn't set it.

When segmenting the output from the mpegts muxer, one can
now set this option when cutting to a new segment, to make sure
the next segment starts with PAT/PMT/SDT.

This is so that TS fragments produced by
http://code.google.com/p/httpsegmenter/
would be compatible with JW Player.
A new member variable prev_payload_key was added to MpegTSWriteStream
to help detect transition from non-key to key frame, so that
PAT/PMT would not be produced for every keyframe in intra-only videos.

With the current default PES packet size, and very small audio bitrates,
audio packet duration gets too long. For players, which wait for a whole
audio packet (or more) it takes a very long time to start playing sound.
For 24kbps audio, one PES packet is about 1 second long. On Motorola STBs,
we observe about 3 second delay before the playback starts with the
default setting.

Do not assume the audio packets being always smaller than
DEFAULT_PES_PAYLOAD_SIZE.
