Makes it robust against adding fields before it, which will be useful in
following commits.
Majority of the patch generated by the following Coccinelle script:

so using an FF-prefix is more appropriate.

Libav, for some reason, merged this as a public API function. This will
aid in future merges.
A define is left for backwards compat, just in case some person
used it, since it is in a public header.

previously quality could only be set through qscale/global_quality but the scale
was inverted. Using a separate option avoids the confusion from qscale working
backward.
