This commit does for AVOutputFormat what commit
20f9727018 did for AVCodec:
It adds a new type FFOutputFormat, moves all the internals
of AVOutputFormat to it and adds a now reduced AVOutputFormat
as first member.
This does not affect/improve extensibility of both public
or private fields for muxers (it is still a mess due to lavd).

move comments for the size of SDP_MAX_SIZE here:
Some SDP lines, particularly for Realmedia or ASF RTSP streams,
contain long SDP lines containing complete ASF Headers (several
kB) or arrays of MDPR (RM stream descriptor) headers plus
"rulebooks" describing their properties. Therefore, the SDP line
buffer is large.
The Vorbis FMTP line can be up to 16KB - see xiph_parse_sdp_line
in rtpdec_xiph.c.

For muxing, it accepts
both 0 and AV_NOPTS_VALUE. For demuxing, it will present
AV_NOPTS_VALUE when start_time_realtime is unknown.

This eases adding options that are common for both. The
AV_OPT_FLAG_EN/DECODING_PARAM still indicates whether they belong
to the muxer or demuxer.

It doesn't look fit to be a part of the public API.
Adding a temporary hack to ffserver to be able to use it, should be
cleaned up when somebody is up for it.
For mpegts in RTP, there isn't a direct mapping between RTSPStreams
and AVStreams, and the RTSPStream isn't ever stored in
AVStream->priv_data, which was earlier leaked. The fix for this
leak, in ea7f080749, lead to
double frees for other, normal RTP streams.
This patch avoids storing RTSPStreams in AVStream->priv_data, thus
avoiding the double free. The RTSPStreams are always available via
RTSPState->rtsp_streams anyway.
Tested with MS-RTSP, RealRTSP, DSS and mpegts/RTP.
