The function now accepts an existing buffer to avoid unnecessary allocations,
as well as only reporting the needed amount of bytes if you pass a NULL pointer
as input for data.
For this, both parameters become input and output, as well as making data
optional. This is backwards compatible, and as such not breaking any existing
use of the function in external code (if there's any).

The dynamic metadata contains data for color volume transform -
application 4 of SMPTE 2094-40:2016 standard. The data comes from
HEVC in the SEI_TYPE_USER_DATA_REGISTERED_ITU_T_T35.

It is only (mis-)used to set the dsp fucntions clear_block(s). But
these functions always work on 16bits-wide elements, which make
the parameter useless and actually harmful, as it causes all content
on more than 8-bits to not use accelerated functions.

This allows masking CPU features with the -cpuflags avconv option
which is useful for testing different optimisations without rebuilding.

This reduces the number of false dependencies on header files and
speeds up compilation.

Consistently apply this rule: the guard name is obtained from the
filename by stripping the leading "lib", converting '/' and '.' to
'_' and uppercasing the resulting name. Guard names in the root
directory have to be prefixed by "FFMPEG_".
