There are lots of files that don't need it: The number of object
files that actually need it went down from 2011 to 884 here.
Keep it for external users in order to not cause breakages.
Also improve the other headers a bit while just at it.

This commit does for AVOutputFormat what commit
20f9727018 did for AVCodec:
It adds a new type FFOutputFormat, moves all the internals
of AVOutputFormat to it and adds a now reduced AVOutputFormat
as first member.
This does not affect/improve extensibility of both public
or private fields for muxers (it is still a mess due to lavd).

Removing explicit avio_flush() calls helps us to buffer more data and avoid
flushing the IO context too often which causes reduced IO throughput for
non-streamed file output.
The user can control flushing behaviour at the end of every packet using the
-flush_packets option, the default typically means to flush unless a
non-streamed file output is used.
Therefore this change should have no adverse effect on streaming, even if it is
assumed that a new packet has a clean buffer so small seekbacks within the
output buffer work even when the IO context is not seekable.

Only the first element of the array is used currently, the other
elements are in preparation for a new muxer calculating multiple
hashes.
Also move alloc/init code from the write_header() functions to
dedicated init() functions, and the cleanup code from the
write_trailer() functions to dedicated deinit() functions.
hash_free() and framehash_free() turn out to be identical here,
but will differ in the subsequent commit, so they are not consolidated.

Use macros to ease the proliferation of identical options to

Note that convergence_duration had another meaning, one which was in
practice never used. The only real use for it was a 64 bit replacement
for the duration field. It's better just to make duration 64 bits, and
to get rid of it.

Due the crappy issue of classes having to be unique,
we cannot even have classes with identical content,
otherwise the compiler or linker is free to merge them,
giving the same result and bugs as if we had only one class.
Besides redesigning the option code to handle this correctly
I see only the option of requiring any two classes to have
different names, but the list of requirements for
AVClasses is getting kind of unmanageable.

Right now those muxers use the default timebase in all cases(1/90000).
This patch avoid unnecessary rescaling and makes the printed timestamps
more readable.
Also, extend the printed information to include the timebases and packet
pts/duration and align the columns.
Obviously changes the results of all fate tests which use those two
muxers.
AVFMT_NOTIMESTAMPS for md5, as it ignores the timestamps.
AVFMT_VARIABLE_FPS for framemd5, as it prints dts.
-vsync 0 for the vp8 test is needed because with vsync 2 the timestamp
guessing code gets confused by an altref frame that is never displayed
and drops a frame later.
In the name of consistency:
put_byte -> avio_w8
put_<type> -> avio_w<type>
put_buffer -> avio_write
put_nbyte will be made private
put_tag will be merged with avio_put_str

This also lists the objects from those two libraries as internal (by adding
the ff_ prefix) so that they can then be hidden via linker scripts.
